‘My Eyes Were So Swollen I Couldn’t Even Open Them’

Onemanda Preisinger is nervous about her child’s impending 13th birthday celebration. It’s not due to the typical reasons related to a house full of loud adolescent children, but because it’s the first time she will reveal her new face to acquaintances and relatives. “Clearly I’m going to inform everyone as they come in, ‘For your information, this is not how I appear,’” states the 30-year-old property agent from Southern Florida.

Her appearance is, admittedly, a somewhat surprising – her face swollen and unnaturally tightened, as though secured by heavy-duty tape. Her new – and she’s eager to emphasize, temporary – look is the outcome of half a dozen aesthetic surgeries, such as an endoscopic mid-facelift, performed by a surgeon in Istanbul, Turkey, last month. “My poor husband teared up when he viewed me for the first time because I couldn’t even unseal my eyes. That indicates puffy I was,” she explains to me via online call from her house. “I had to tell him: ‘Babe, I’m fine, I’m not in pain. I just look like someone attacked me.’”

Increasing Popularity of Early Facelifts

Based on plastic surgeons, Preisinger is among a growing number of people choosing to have a facelift in their twenties and thirties – significantly before a decade before most doctors’ usual candidate age of forty to sixty. (Although most surgeons are eager to highlight the professional guideline of: “We address heredity, not age.”) “I have individuals in their late twenties asking for facelifts,” states based in London cosmetic surgeon Georgios Orfaniotis, a former NHS consultant who specialises in the facial area. “It’s a major surgery, and I’m a bit concerned when I observe individuals opting for it as a personal preference.”

Understanding the Facelift Procedure

A rhytidectomy, alternatively called a facelift, elevates the tissues in the face that begin to droop as we grow older. “Human faces are built a bit like layers of an onion,” says Kent-based face specialist Marc Pacifico. “Skin and fat on the top, then a stratum of connective tissue called the SMAS. Think of the SMAS as the soft tissue skeleton of the face. And that is what ages us; that is what loosens and sags and drags the skin down with it.”

Previously reserved of affluent seniors, “filler fatigue” – when excessive use of dermal fillers stretches the face and causes looseness in the skin – combined with the common adoption of slimming medications, cut-price medical tourism, and the advancement of novel, famous-figure-promoted operative methods are attracting a new kind of customer towards this invasive surgery. Statistics indicate an eight percent rise in facelifts over the last year in the United Kingdom; while a survey revealed that the percentage of youthful candidates is growing, with one-third of procedures now performed on patients aged 35-55.

“People are asking now to appear as the best version of themselves and naturally the methods are evolving,” says Orfaniotis. Currently, the trending procedure is the deep plane facelift, a technique evangelised by figures including Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Deep Plane Technique

While a more traditional facelift entails elevating the superficial layer, the comprehensive lift releases the facial ligaments beneath it, allowing doctors to restructure the face by repositioning the underlying layers, and avoiding the taut, pulled appearance occasionally caused by more traditional techniques. “We’re not putting stress on the dermis because we’re targeting deeper, to the deeper tissue,” says prestigious London plastic surgeon Rajiv Grover, who specialises in the deep plane technique. “It is the underlying layers that is adjusted. The skin comes with it as a passenger.” Elevating the entire soft tissue layer as a unified block allows for a authentic-appearing and longer-lasting outcome. It also means that the procedure is no longer being used for the express purpose of anti-ageing – or revitalization, as the cosmetic field terms it – but for “beautification” and contouring, too. “We receive many inquiries from patients aged 28 to 35 for this reason,” says Grover.

Personal Journey

Preisinger had never planned on undergoing a facial lift – at least not at the age of 30. But long-term using injectable gels in an effort at “symmetry correction” meant that by the time she entered her late 20s, she was “botched”. “I didn’t do this because I wanted to look youthful,” she says. “I underwent it because the injectable harmed my face. I regret ever using it. If I had avoided the filler, I don’t think I would be in this situation currently.”

Whether dermal fillers ever fully dissolve has long been a point of contention in the cosmetic field. “Studies have shown that they seldom fully dissipate,” says Grover, “but they move and can block drainage pathways. One of the contributors to what we term ‘pillow face’ is also the reality that in a sense, the face gets slightly fluid-filled because its capacity to drain bodily fluid has been diminished.” Though research into the topic is preliminary, warnings on dermal fillers’ potential impact on the body’s drainage have been released, and additional studies initiated. An esteemed facial plastic surgeon, speaking on the condition of anonymity, says he would never use fillers in a patient because of the dangers they present. “Many surgeons don’t want to discussing this, because the manufacturers who make filler can be pretty aggressive.” He’s heard stories, he adds, of plastic surgeons being landed with court orders after expressing worries.

Decision and Procedure

For Preisinger, once she started injecting filler, she felt as if she needed to continue replenishing it – particularly when it started to migrate to different areas of her face. She was just 28 when a nearby cosmetic doctor in the state suggested that a face and neck lift might be what she required to finally step off the cycle of treatments. Two years later, she found herself choosing between a list of suggested accommodations in Istanbul, sent to her by the patient coordinator at her doctor’s clinic.

Initially, Preisinger had scheduled an upper and lower blepharoplasty – better known as blepharoplasty – but her surgeon advised her that a facelift was the right solution for the laxity she was experiencing in her face. She booked a forehead lift, a mid-facelift and a jawline surgery. The eve of her operation, one day after she’d arrived alone in the country, she decided to add a mouth enhancement. “Then he said, ‘We’re going to elevate the under-eye bags.’ Then he said, ‘I think if we extract some buccal fat, it will contour your face.’ So I ended up including additional surgeries,” she recalls.
